CareSelect: Imaging Decision Support Scoring Upgrade

On Monday, November 11, the indications on advanced imaging orders will be updated automatically as part of the scheduled upgrade of the decision support tool CareSelect. The purpose of the clinical decision support mechanism for advanced imaging orders is to be compliant with the CMS PAMA (Protecting Access to Medicare Act) with guidelines to consult and adhere to appropriate use criteria (AUC) and Medicare billing regulations.

  • The decision support tool assists ordering providers in making the most appropriate decisions for imaging based on a patient's specific clinical condition. This regulation goes into effect on January 1, 2020, commencing the testing period for the AUC program.

Changes with this Upgrade:

This upgrade includes refinement of content without any changes to the ordering workflow steps. You will continue to only see the BPA if the indication on the advanced imaging order is scored as Low (score of 1-3) or Marginal (score of 4-6). With this upgrade, echocardiograms will be evaluated as in scope with decision support.

The most noticeable change is that the BPA may, at times, show the score category (Red, Yellow, or Green) without a numeric score for specific indications. The scoring algorithms are based on multiple clinical sources - primarily the American College of Radiology (ACR), which provides numeric scores. Other clinical scores - including the American College of Cardiology (ACC) and the National Comprehensive Cancer Network (NCCN) may provide the score category without an actual number for certain indications. The scoring scale of 1-9 remains the same across all advanced imaging orders:

  • Red = Low score of 1-3, Rarely Appropriate
  • Yellow = Marginal score of 4-6, May Be Appropriate
  • Green = High score of 7-9, Appropriate or Recommended

Additional Tip: The SEARCH window can be used to type in a keyword, which will provide a list of matching indications. The search feature also functions with synonyms.
