Patient Admission Notifications
Regulatory requirements require documentation in the medical record that the medical provider and caregiver of choice are notified of a patient's admission at the time of admission. If the patient requests notification, the name, phone number, and status of the notification need to be documented. If the patient is incapacitated (UTA) at the time of admission, a social work consult must be placed for family member and provider notification. To meet this requirement, required documentation has been added to the Nursing Admission Navigator and the Case Management Navigator. The additions are documentation of the patient's desire to notify a family member of the admission and/or to notify the patient's primary care provider (PCP), contact name, phone number, and communication status.
