Required Documentation: CHG Baths For Central Line Patients

Documentation of CHG bathing is now added to the list of required documentation for patients. This change will occur February 13, 2023

The task appears on the Nursing Brain and the Brain sidebar within the patient's EHR under required shift documentation. The task is assigned for 9:00 a.m. and is due within 24 hours. To complete documentation, nursing can follow the flowsheet link in either location to navigate to the Daily Cares/Safety Flowsheet. Documentation occurs in the Hygiene section on the Skin Care now. Selecting CHG solution bath or CHG cloth bath completes the documentation requirement and removes the task from the Brain. 

 



Required Documentation: Patient Belongings/Valuables Returned Upon Discharge

Effective Monday, February 13, 2023, a reminder to retrieve any patient belonging stored or sent to the hospital safe since the hospital admission now appears as required documentation on the Brain under the Discharge required documentation. The blue link on the reminder takes the user to the Discharge Navigator to complete the documentation. On the Discharge Navigator, scroll to Belongings Returned, and mark the appropriate answer. 



Mechanical Prophylaxis Flowsheet Change

A change to the Daily Cares/Safety Flowsheet regarding mechanical prophylaxis results in flowsheet documentation consisting of only 3 rows specific to VTE Mechanical Prophylaxis (instead of the current 5). Expect to see this change Monday, February 13, 2023. Previous rows with documentation can still be viewed, but will no longer be active for continued documentation. 

New flowsheet rows: 

  • VTE Not Ordered 
  • VTE Prophylaxis 
  • VTE Prophylaxis Intervention
