Medication details at discharge are important to ensure that the patient is informed of medications. As a regulatory requirement, when a patient is given discharge instructions the recommended time to take the medication and information of when the last dose was received must appear on the document. To assist nursing in this process, the discharge navigator has been updated. In the PatientInstructions topic of the DischargeNavigator the patient is provided with updated patient information in the After Visit Summary. The Add Med Details section provides information about a patient's medications, the medication generic and common names, dosing instructions, comments and the time of day to take the medication. As a TJC regulatory requirement, when a patient is given discharge instructions, the recommended time to take the medication and information of when the last dose was received must appear on the document.
