The Care Coordination Note can be used to communicate with other providers across encounters. The Care Coordinator note is not shared through MyChart and can be seen across specialties. 

Once a note has been created, it will follow the patient across encounters and be available in the following locations: 

• TreatmentTeam (can enter a new note and/or edit existing notes from here) 
• ProblemList (cannot add a new note from here but can edit an existing note) 
• SnapshotReport>>CareTeamandCoordination section (can view an existing note from here, but cannot enter a new note or edit an existing note. However, you can launch the Treatment Team activity from here)
