Possible Epic Patient Safety Issue

During the Diagnosis Association for Orders, if updates are made to a diagnosis with additional specificity from the Problems List, those updates may not be saved to that particular diagnosis but to a DIFFERENT diagnosis instead.

If the clinician associates the diagnosis from the Problem list to the orders using the Diagnosis Associate grid in outpatient encounters, there is a possibility that behind the scenes it files to a different record’s problem list.  

To avoid triggering the issue, while working in the chart, use the Visit Diagnosis list instead of the Problem list Diagnosis to add the Diagnosis to the Order.

Epic is expected to deliver a fix to us this week.  We will notify everyone when the fix has been installed.
