Required Fields and Update to CHIP and VFC Question for Immunizations

Immunization Required Fields
The Date, Time, Lot #, Manufacturer, NDC, Expiration Date, Site and Route are all required for documentation of administration of an immunization.

Options to indicate if a vaccine is state funded have been combined into one question. VFC and CHIP can be indicated by the appropriate responses listed under the question “vaccine funding source”, which is shown below. If a patient is not eligible for state funded vaccines, the no-not VFC/CHIP eligible selection should be used.
