Provider Points: Premie BiliRecs Tool Now Available in Epic

The new "Premie BiliRecs" Tool (PBR) is now available in Epic, providing clinical support for treatment of indirect hyperbilirubinemia in pre-term neonates from 27 weeks and 0 days through 34 weeks and 6 days post-menstrual age.

Because phototherapy and exchange transfusion thresholds are lower at early postnatal age, the tool is intended for use in infants greater than 48 hours of age. Ultimately, expert clinical reasoning should guide the management of indirect hyperbilirubinemia in preterm neonates of any postnatal age.

While it may sound similar, the PBR Tool is different than the existing BiliTool hyperlink:

  • PBR Tool: Premature babies
  • BiliTool: Babies greater than 35 weeks gestational age

The PBR Tool evaluates based on the patient's most recent bilirubin result. Even if you select an older bilirubin, it will still evaluate based on the most recent result.

Finding the PBR Tool:

  • The link will only appear in the "Lab Results" report under Chart Review
  • It should only appear if the child is at least 48 hours old
  • It should be available for both inpatient and ambulatory settings
  • It should not appear if the child is less than 27 weeks or greater than 35 weeks post-menstrual age

Using the PBR Tool:

Once you click the link, you will be taken to a screen that looks like the image below. If the patient does not fit the criteria, you will likely receive an "error" screen.

Remember, the PBR Tool will evaluate the most recent Bilirubin - not necessarily the Bilirubin that you are reading when you click the link.
