Health Requirements – NEPAR

Submission of non-employee health documents will be captured through a REDCap survey after non-employee completes the eNEPAR initial submission. The email entered on the survey by non-employee will receive a link the following business day from NEPARHealthPortal@umc.edu.

Please note:

• The health requirements are to be met at your own expense.
• Provide documentation for these vaccines and TB screening at least two weeks before the start date.
• Requirements may vary based on your role, location, or duties.

Health Requirements

• MMR – 2 doses or (+) titers for measles, mumps, rubella 
• Varicella – 2 doses or (+) titer 
• TDAP – 1 Tdap on file then 1 TD every 10 years 
• Hepatitis B – 3 doses and/or (+) titer or declination form 
• Influenza – 1 every flu season 
• TB test – 2 TB skin tests or 1 TBGQ within 90 days of hire
