Tobacco-related Cancers in Mississippi, 2003-2023
Smoking, exposure to second-hand smoke, and use of other tobacco products are a modifiable risk factor associated with the development of certain cancers. According to the Behavioral Risk Factor Surveillance System (BRFSS) for 2024, 13.8% of Mississippi adults report being current smokers, 23.8% report being former smokers, and 4.3% of Mississippi adults report using smokeless tobacco. Mississippi's rate of current smoking among adults is 12th highest in the nation including the District of Columbia and Puerto Rico.1 Tobacco use is associated with cancers of the lip, oral cavity, pharynx, stomach, colon and rectum, pancreas, trachea, lung and bronchus, cervix, kidney and renal pelvis, urinary bladder, esophagus, liver, and larynx.2 Tobacco use is also associated with the development of acute myeloid leukemia.2 Cancers of the lung and bronchus are the most frequent types of cancer diagnosed in those using tobacco.2 Lung cancer screening is an effective tool for detecting cancers of the lung and bronchus early.3 According to the 2024 BRFSS, only 18.8% of Mississippians eligible for screening based on the United States Preventive Services Task Force guidelines have been screened. That is the 8th lowest rate of screening in the nation including the District of Columbia and Puerto Rico.1 Below are graphs of the trends in tobacco-related cancers over the period 2003 to 2023 by race and sex with a description of the trends occurring in each group both for the full time period and for the most recent period between 2019 and 2023. All analysis was done using SEER*Stat software4 and SEER's Joinpoint Regression software5.

While lip, oral cavity, and pharynx cancer is associated with tobacco use, it is also associated with another modifiable risk factor, excessive alcohol use. Cancers of the oropharynx may also be associated with Human Papillomavirus (HPV) infection. Males had significantly higher rates of lip, oral cavity, and pharynx cancers than females. Over the period from 2003 to 2023, only black males and white males experienced a significant change in incidence rates. The rate for black males decreased annually by 1.69%. The rate for white males increased annually by 0.73%. The rate for black females was observed to be decreasing at an annual rate of 0.62%. The rate for white females were observed to be increasing at annual rate of 1.33%.
For the latest five-year time period of 2019 to 2023, white males and white females had a similar trend to that of the overall time period of 2003 to 2023. The rate for white males increased significantly at an annual rate of 1.36%. Though not statistically significant, the rate for white females was increasing at a rate of 4.54% annually. The rates for black males and black females were opposite of the trend for the overall time period. The rate for black males was observed to be decreasing at an annual rate of 3.10%, and the rate for black females was observed to be increasing at an annual rate of 0.75%.

While all stomach cancers may be associated with tobacco use, cancers occurring in the gastric cardia (upper portion of the stomach next to the esophagus) may also be associated with obesity. Between 2003 and 2023, black males had significantly higher incidence rates for stomach cancer than all other groups except in 2015 and 2020. The rate for stomach cancer in black males decreased significantly between 2003 and 2023 at an annual percent change of 1.74%. No other group had any significant change over time. Black females had an observed increase of 0.72% annually between 2003 and 2023. White males had a small increase of 0.12% annually, and for white females, the observed change was a 0.64% annual increase.
For the latest five-year period from 2019 to 2023, none of the trends were statistically significant. Black males saw an observed annual increase of 3.10%, while black females saw an observed increase of 4.92% annually. The annual increase for white males was 5.89% in contrast to the relatively flat trend for the full time period. White females had an observed annual increase of 3.71%.

While pancreatic cancer is associated with tobacco use, it is also associated with another modifiable risk factor, obesity. Pancreatic cancer rates increased significantly between 2003 and 2023 for females, overall. White females experienced annual significant increase of 3.53% between 2003 and 2011 but then saw and observed 0.17% decrease between 2011 and 2023. Overall, white females had a significant average annual increase of 1.29%. The rate for black females increased a significant 1.87% annually between 2003 and 2020 and then decreased at an observed 7.06% annually between 2020 and 2023. Overall, black females had an observed 0.47% annual increase for the full time period. For males, the average annual percent change between 2003 and 2023 was a significant increase of 1.54%. Black males had an observed increase of 0.41% annually. White males experienced a significant increase between 2003 and 2015 of 3.04% annually, but then experienced an observed decrease annually between 2015 and 2018 of 6.39%. Finally, between 2018 and 2023, the rate for white males increased at observed annual rate of 2.95%. This translates into a statistically significant average annual increase of 1.54% over the full time period.
For the latest five-year period of 2019-2023, the trends were increasing for all groups except black females who saw a decreasing trend. White males had an observed increase of 4.04% annually. For black males, the most recent trend observed is an increase of 1.46% annually, and for black females the observed trend for the most recent five years was a 4.31% decrease annually. White females had an observed annual increase of 2.63%.

Males had significantly higher rates of trachea, lung, and bronchus cancer than females. For most years, black males had significantly higher rates than white males. White females had significantly higher incidence rates than black females. The rates for both white males and black males decreased at a significant average annual rate between 2003 and 2023. For white males, the rate increased 2.37% annually between 2003 and 2007. Then, between 2007 and 2023, the rated decreased significantly 2.56% annually. This translates to a significant average annual percent decrease of 1.59%. For black males, between 2003 and 2018, the rate decreased at an observed rate of 1.58%. Then, between 2018 and 2023, the rate decreased significantly at an annual rate of 2.56%. Thus, the average annual percent decrease was a statistically significant 1.59%. Black females had no significant change in their rates with an annual decrease of 0.25%. For white females, between 2003 and 2017, the rate significantly increased 0.78% annually. Then, between 2017-2023, there was an observed decrease of 1.63% annually. This translated to a relatively flat increasing trend over the full time period of 0.05% annually.
Compared to the trends for the full time period from 2003 to 2023, white males, black males, and black females had similar trends for the most recent five-year time period of 2019 to 2023. The rate for white males was observed to decrease at a rate of 2.81% annually, and the rate for black males decreased significantly at a rate of 3.40% annually. For black females, there was an observed increase of 0.13% annually. For white females, the rate for the latest five years is increasing at an observed rate of 0.18%.

While cervical cancer is associated with tobacco use, most cervical carcinomas are associated with Human Papilloma Virus (HPV) infection. For most years, there was not a statistically significant difference in the incidence rates of cervical cancer between white females and black females, though black females did have higher observed rates through 2015. During the period between 2003 and 2023, the incidence rate for black females decreased at a significant rate of 2.47% annually. In contrast, the observed change in the incidence rate for white females was a 1.78% increase annually, though this change was not statistically significant. The incidence gap between black females and white females has been closed in the most recent years as a result of the significant decrease in rates for black females and the observed increase in rates for white females. Between 2019 and 2023, in contrast to the full time period, black females had an observed increase of 8.11% annually. The rate for white females decreased 1.49% annually between 2019 and 2023, though this change was not statistically significant.

While kidney and renal pelvis cancers are associated with tobacco use, kidney cancer is also associated with another modifiable risk factor, obesity. Males had significantly higher rates of kidney and renal pelvis cancer than females. The rates for males were similar over time. Rates for females were similar for all years except 2009, 2010, 2016, 2017, and 2019 where black females had significantly higher incidence rates. Rates for all groups increased between 2003 and 2023. The rate for white males increased significantly 2.13% annually, and the rate for white females increased significantly 1.84% annually. The rates for black males increased significantly at a rate of 8.64% annually between 2003 and 2008. Then, between 2008 and 2023, the rate of increase for black males was observed to be 0.15% annually. This translates into a significant annual increase of 1.21% over the full time period. For black females, the incidence rate increased significantly at a rate of 4.45% annually between 2003 and 2017, followed by a significant annual decrease of 5.73% between 2017 and 2023. This translates to an observed annual increase of 1.29% for black females for the full time period.
For the latest five-year period of 2019 to 2023, white males, and white females had observed annual increases of 3.39% and 7.12%, respectively. Black males and black females had observed annual decreases of 2.67% and 7.78%, respectively.

Males had significantly higher rates of urinary bladder cancer than females. Also, white males had significantly higher rates than black males. The observed annual percent change between 2003 and 2023 for all groups was relatively small. Black males had a 0.04% increase annually in their incidence rates, and black females had a 0.36% increase annually. For white females, the annual decrease was 0.53%. For white males between 2003 and 2021, the decrease was observed to be 0.04%. However, from 2021 to 2023, white males had a significant decrease of 9.03%. For the overall time period between 2003 and 2023, white males had an observed decrease of 0.98%.
From 2019 to 2023, all groups experienced a decreasing trend. White males had an observed annual decrease of 3.53%, and white females had a significant annual decrease of 3.14%. Black males had an observed annual decrease of 2.22%, and black females had an observed annual decrease of 5.72%.

While esophageal cancer is associated with tobacco use, it is also associated with another modifiable risk factor, excessive alcohol use. Additionally, adenocarcinoma of the esophagus can be associated with obesity. Males had significantly higher rates of esophageal cancer than females. The rates were similar by race for females and similar for males beginning in 2013. The annual percent change over the period from 2003 to 2023 for black males was statistically significant. For black males, the annual percent decrease was 3.17%. The rate for black females decreased at an observed annual rate of 1.13%. In contrast to black males and females, the rates for white males and white females increased, those the changes were not statistically significant. The rate for white females increased 1.52% annually, and the rate for white males increased 0.42% annually.
For the latest five-year time period of 2019 to 2023, white females and black females had a similar trend to what was observed for the full time period of 2003 to 2023. The trend for 2019 to 2023 for white females was an annual increase of 2.23% and for black females was an annual decrease of 4.47%, neither of which were statistically significant. For black males, the trend for the most recent time period was similar of the full time period from 2003 to 2023 but not statistically significant. For 2019-2023, black males experienced an observed decrease of 6.32% annually. White males, opposite of the full time period, experienced a decreasing trend, though the change was not statistically significant. For 2019-2023, the rate for white males decreased at a rate of 0.13% annually.

While liver cancer is associated with tobacco use, it is also associated with two other modifiable risk factors, excessive alcohol use and obesity. Males had significantly higher rates of liver cancer than females. The rates were similar between the races for females and for males prior to 2012. Beginning in 2012, white males had significantly higher rates that black males for many of those years. For white males, the incidence rate increased significantly between 2003 and 2016 at a rate of 5.02% annually. Between 2016 and 2023, the rate for white males decreased at a rate of 2.80% annually, though this change was not statistically significant. The rate for white females showed an annual decrease of 11.38% from 2003 to 2006, but then showed a significant increasing trend from 2006 to 2023 with an annual increase of 3.39%. The observed annual percent increase for black females was 1.07%. Between 2003 and 2017, the rate for black males increased at an observed rate of 2.31%, but then showed a significant decrease of 8.22% annually between 2017 and 2023.
For the latest five-year period of 2019 to 2023, trends were decreasing for all groups except white males. None of the changes were statistically significant. For white males, the trend for the last five-year period was an increase of 0.85% annually, and for black males, the most recent trend observed was a decrease of 3.28% annually. For black females, the decrease was 2.74% annually, and for white females, the decrease was 0.28% annually.

While laryngeal cancer is associated with tobacco use, it is also associated with another modifiable risk factor, excessive alcohol use. Laryngeal cancer is a relatively rare cancer. Thus, the rates could not be broken down by both race and sex. This graph only displays the rates by race. Over the period from 2003 to 2023, the annual decrease for the white population was 1.05%, though not statistically significant. For the black population, between 2003 and 2007, there was an annual observed increase of 7.73%. Between 2007 and 2023, the black population experienced a statistically significant decrease of 3.20% annually. For the latest five-year time period of 2017 to 2023, the black population and white population saw decreasing trends. These trends were only statistically significant for the black population. For the black population, the trend was an annual decrease of 5.21%, and for the white population, the trend was an annual decrease of 1.81%.

Acute myeloid leukemia is a relatively rare type of cancer. Rates over the period from 2003 to 2023 remained relatively stable for the black population with an observed annual increase of 0.48%. The white population saw a significant increase over this time period of 1.94% annually. The rates were similar between groups for most years.
For the latest five-year time period from 2019 to 2023, the trend for the white population was opposite the trend for the full time period from 2003 to 2023 but was not statistically significant. The white rate decreased between 2019 and 2023 annually 2.07%. The rate for the black population was also observed to decrease 1.88% annually.

While colorectal cancer is associated with tobacco use, it is also associated with other modifiable risk factors, obesity (both colon and rectum cancers), alcohol use (both colon and rectum cancers) and lack of physical activity (only colon cancer). Black males had significantly higher rates of colorectal cancer than all other groups except for in years 2003, 2004, and 2022 where their rates were similar to those of white males. Colorectal cancer rates decreased significantly in all groups for the period between 2003 and 2023. White females had a significant decrease of 0.56% annually, and white males had a significant decrease of 1.24% annually. Black females experienced an annual observed decrease of 0.76% between 2003 and 2011, followed by a significant decrease annually of 2.70% between 2011 and 2023. These patterns translate into a significant average annual percent decrease of 1.93% for black females over the full time period. Black males had a significant decrease of 0.97% annually between 2003 and 2023.
For the latest five-year period of 2019 to 2023, white males and white females experienced an observed increase of 0.38% and 1.36%, respectively. Both black males and black females had a similar pattern in the latest five years as they had in the full time period, though the changes were not significant. The rate for black males was observed to decrease at an annual rate of 0.29% between 2019 and 2023, while the rate for black females was observed to decrease at an annual rate of 1.53%.
Definitions
Age Adjusting: A statistical method that allows comparisons of populations that take into account age-distribution differences between the populations. The 2000 U.S. standard population is used and applied to all of the time periods being considered. This assures that the rates do not reflect differences in the age distribution of the population.
Annual Percent Change (APC): The average annual percent change over several years. It is used to measure the change in rates over time. Calculating the APC involves fitting a straight line to the natural logarithm of the data when it is displayed by calendar year.
Statistical Significance: This is a mathematical measure of the difference between groups. A difference is said to be statistically significant if it is greater than what might be expected to happen by chance alone 95% of the time. Rate ratios were used to assess the statistical significance between groups.
