Advanced Planning Tools for Persons with Dementia

The Mississippi Dementia Advance Planning Project introduces a tool designated to foster essential conversations about end-of-life planning. The Dementia Values and Priorities Tool is designed to help persons living with dementia communicate their wishes regarding future care. This presentation will introduce the tool and explore questions related to the changes commonly seen in the progression of dementia. The tool provides users with a document that can be added to their existing advance directive and shared with others.
