If you have diabetes, heart disease, high blood pressure, kidney disease, or other chronic illness, you probably know that strictly following your doctor’s orders can make a big difference in how you feel and how healthy you are. But following your care plan to the letter isn’t always easy. That’s where remote patient monitoring (RPM) can help.
When your provider refers you for the RPM program from UMMC, he or she is giving you a helping hand to stay focused on your health.
Through this four- to six-month program, you’ll receive:
- A personalized care plan tailored to your health challenges and a tablet computer you can use during the program.
- Education and advice from UMMC specialists about your condition and how to improve your health.
- Regular check-ins with a UMMC registered nurse through messages and video chats on your tablet computer to keep you on track and catch any potential problems early.
- Support, encouragement, and guidance so you can learn to monitor and manage your health on your own.
With a provider referral, RPM care is usually covered by your insurance or Medicaid plan.
Our RPM program has been shown to help patients better manage their blood sugar and blood pressure, learn more about how to control their conditions, and reduce the number of times they had to go to the hospital for those conditions.
This program requires a provider referral.
