UMMC Qualtrics Nomination Survey

The last day to complete the UMMC Qualtrics nomination survey will be Oct. 18, 2024.   

Award:  

$5,000; a beautiful plaque featuring a specially cast medallion; and a trip to Houston to receive the award at the annual Installation of Officers Banquet, customarily held in late January (Friday, Jan.17, 2025) 

Criteria:  

Judging Criteria Breakdown 

75% will be based on: 

• Practice 
• Teaching/Education experience 
• Research/Scholarship effort evidence Medical Society Service 

25% will be based on: 

• Healthcare Service and Advocacy 
• Includes Medical Society Service, Government Service, and Service to Allied Health Professions  
• Health Promotion and Education 
• Includes Health Promotion and Disease Prevention, Medical Humanities 
• Resource Development and Support 
• Includes Medical Libraries and Philanthropic Activity 

Guidelines for Entry: 

• A nomination letter for the candidate should detail their personal and professional achievements, specifically highlighting aspects of the ‘Judging Criteria Breakdown’ that reflect the nominee’s character as a Compleat Physician.  
• Current full curriculum vitae 
• Acknowledgement that the candidate will be available and present for the Award ceremony in Houston, Texas, on Friday, Jan.17,2025 - only the winning recipient needs to attend
