UMMC Qualtrics Nomination Survey
The last day to complete the UMMC Qualtrics nomination survey will be October 25, 2024.
Award:
Awards, including a suitable recognition presentation piece, will be made by the AMA upon the selection of inners by an outstanding panel of judges. Awards will be presented at a recognition dinner in Washington, D.C.
Criteria:
The criteria to be observed in selecting these outstanding individuals shall include one or more of the following:
- Contributed greatly to the public health through elected and career government service
- Active leader in their area of specialty
- Has high personal integrity
- Promoted the art and science of medicine in or through government service
- Developed a special project that contributed to the public health of a given community or special population
Guidelines for Entry:
- Complete the nomination form – included is a section for a brief biographical statement about the nominee
- Explanation of why you believe your nominee should receive the American Medical Association for Outstanding Government Service (no more than 500 words)
- Additional materials may include: letters, testimonials, news clippings, pamphlets, etc. Please be aware that all submitted materials become property of the American Medical Association and will not be returned.
